COVID-19 VACCINES

VACCINATION CLINIC- APRIL 3, 2021

Rite Aid Pharmacy will be hosting a COVID-19 Vaccination Clinic at Eben-Ezer Church of God from 9:00 AM - 3:00 PM on Saturday, April 3, 2021. Here are some important details:

  • Please bring your insurance card and photo ID to your appointment. 

  • If you cancel your appointment or are no longer able to attend last minute, please email ecogwebmaster@gmail.com and let us know as soon as possible. 

  • There are six available spot for each 15-minute time-slot below. Please sign each individual up separately. 

  • The second dose of the vaccine will be administered on Saturday, April 24 at ECOG.

  • REGISTRATION WILL CLOSE ON THURSDAY, APRIL 1.
